The Henryetta Lions Club will be taking Christmas Basket applications on Wednesday, Nov. 20, from 9 a.m. to 5 p.m. at The Briefcase, located at 313 W. Main in Henryetta.
No phone applications will be accepted. Please bring utility bill or driver’s license, proof of income and proof of guardianship of children 13 years old or younger.
Applications will only be accepted Nov. 20, 2024. These applications will be for pick up only. We no longer offer deliveries.
To apply, please be prepared to present the following: • Utility bill or driver’s license showing current address. If landlord pays utilities, please have their name and address.
• Proof of income – Examples of proof of income are an employment pay stub, current bank statement, last year’s tax return, wages and tax statement (W-2 and/or 1099) or a copy of a SNAP letter. You need one of these five items as proof of income.
• Proof of guardianship of children 13 and under – Examples of proof of guardianship are birth certificates, last year’s tax return showing dependents or any legal/court document showing custody or guardianship. One of these three are needed for proof of guardianship. Social security cards alone will no longer be accepted.
• If you do not list a valid phone number on your application, it will be disqualified. We must be able to find you if necessary. Otherwise your basket is forfeited. If you move before the distribution date, please inform us at 918-650-5850.
